Maybe you can add amenity=doctors and healthcare=doctor parallel to be compatible and add healthcare:speciality?

I think this is like highway=bus_stop and public_transport. Amenity=doctor is the old tag and in the Future it is deprecated.

It seems that iD is inserting both keys (ie, when selecting a hospital it inserts amenity=hospital + healthcare=hospital)
A warning for healthcare=yes (asking to use a more specific healthcare facility) should probably be included too in JOSM, when JOSM starts to support healthcare=*

Is anybody able to start with an (external) preset?

I guess centre, clinic, ... and a limited number of specialities would be fine :)

Just a hint - we started rendering it in OSM Carto lately, 3 years after it had been requested:

There is an 3rd party preset already: https://josm.openstreetmap.de/wiki/Presets/Healthcare
And there was further progress at carto https://github.com/gravitystorm/openstreetmap-carto/issues/2938
but since a ehile carto AFAIK dropped currently support: https://github.com/gravitystorm/openstreetmap-carto/pull/3731

Key is documented, used 15.000 times, worldwide, so maybe we could include it? But some values are tricky (example
healthcare=doctorvs.amenity=doctors, etc.)
