You mean for city_limit? This was done on purpose. It is for the case where a city limit sign separates two cities and thus two names exists. Although "alt_name" is used, it is not really an alternative name.
That sounds like a misuse of the alt_name=* tag, it's not meant to be a secondary name that refers to another object but an alternate (and usually unofficial) name, e.g.:
name=Los Angeles alt_name=City of Angels
If the two are supposed to be equivalent they should probably be both in the name=* tag separated by ;

Well, you know - OSM is a database, where not everything follows the rules 100% - that's life :-)
