Steps to reproduce
Open Add TMS URL.
Enter the TMS layer URL.
Under Set custom HTTP headers, enter a Header name and Header value.
Enter a Layer name.
Click Accept directly.
Observe that the layer does not work because the entered HTTP header value has not actually been committed.
Workaround
After entering the HTTP header value, click with the mouse on the next empty row in the custom HTTP headers table.
The entered value is then committed.
Click Accept.
The TMS layer works correctly.
Expected behavior

Clicking Accept should automatically commit the currently edited HTTP header value before creating the TMS layer.
