The preset expects you to have selected the relation itself, not the members of the relation. It needs you to create the relation first, before you tag it with the preset. This is not ideal, but currently how it works.

The way I use a preset to create a turn restriction is as follows:

  1. Open the "list of all relations" window (Alt-R by default).
  1. In the map view, select the node and ways for the restriction.
  1. Click the "+" button in the "list of all relations" window. This creates a new relation.
  1. Click the "Add all primitives..." button in the lower right pane. Set the Roles as needed. Click the OK button.
  1. If you scroll to the top of the "list of all relations" window, you will see the line " (0, 3 members) [id:0]". Double-click on it to select the relation.
  1. From the menu bar, click Presets->Relations->Turn restriction and choose the necessary restriction.
