In JOSM 2221 (and latest 2247), the following OSM files both fail to orthogonalize as expected:

​http://regionaltraffic.co.uk/House1.osm (results in extreme point)
​http://regionaltraffic.co.uk/House2.osm (results in way failure)

In such a house shape I would expect the approximate 45 degree angle to be kept but the other edges used to determine the axis orientation.
